Layout Sign-Up Form

Thank you for purchasing a blog layout! Please fill out the following form and e-mail it to me at hilary.keegan@gmail.com so we can get started on your blog layout. After I have received your completed form, I will verify the details with you and then send you an invoice for half of your total as a deposit. Before filling out this form, please read through my Sign-Up page so that you will understand my blog layout creation process. If you aren't sure what any of the items I have listed below are, please refer to my Pricing page for a full description.

• Your blog address/URL:
• Your blog title:
• Any tagline/subtitle/phrase/description you want me to include in the header:
• Do you have any photos you want me to include in your header? If so, please attach them to the e-mail.
• Which kit would you like me to use? (To see the kits that are available, click on the Kits button at the top of this page)
• Would you like to purchase any Happy Faces? If so, please tell me which ones you would like me to use (indicate row number and column letter). To view all the different choices, see the images at the end of this post.
  
• If you would like to purchase a custom signature, what would you like it to say?
• If you would like to purchase sidebar titles, what would you like them to say?
• If you would like to purchase a sidebar button, is there anything specific you would like me to put on it (usually, I just put the title of the blog on them with some decoration)? Is there a specific size you want your button to be?
• Would you like to purchase a three column layout? If you do no purchase a three column layout, your layout will be two columns.
• Would you like to purchase the custom font option? If so, which parts of your layout would you like me to use the custom font for?
• If you would like to purchase a navigation bar, would you like me to use matching images or text only? What would you like the links in your navigation bar to say?
  
• Do you have anything else you would like me to include in your layout?
• When I am done with your blog layout, I would like to add a mini-screenshot and a link to your blog in my "Portfolio" section of this site. If that is not okay with you, please let me know.
